GREEN DOG BARKERY, LLC
AGREEMENT, WAIVERS, RULES, AND PRIVACY POLICY
Welcome to Green Dog Barkery, our business is located at 1024 Wyoming Avenue, Wyoming, PA
(the “Premises”). Green Dog Barkery is owned and operated by Green Dog Barkery LLC, a
Pennsylvania limited liability company (the “Company”, "we", "us", or "our"). The Premises is dog-
friendly and allows dogs (i) on leash in certain designated areas of the Premises, including the retail store, and (ii) off-leash in certain designated areas of the Premises, including in the indoor and outdoor social area (the “Social Area”).
In order to bring a dog(s) to the Social Area, you, as the individual signing below (referred to as
“you”, "I" , "me", “your” or “Owner”) are required to maintain an active membership with the
Company. You will need to provide information (see further below) required for your dog to become a
member of Green Dog Barkery, along with payment information for membership.
As the Owner of a dog that is a member of Green Dog Barkery, you are also required, among other
things, to represent as to certain facts and statements; to release the Company from liability; to
acknowledge certain risks; to indemnity the Company; and to agree to our Rules, as follows:
Representations
I confirm that my dog(s): (a) is in good health and proper physical condition and does not have any
medical or other conditions that would impair its ability to participate in the Social Area; (b) is current
on Rabies, Distemper (DHPP, DHLPP, DA2PP) and Bordetella vaccinations, and will remain current
throughout the duration of the membership; (c) does not, to my knowledge, have fleas or ticks; (d)
has not demonstrated aggressive behavior and/or bitten any person or animal in the past; (e) does
not, to my knowledge, have any contagious disease; (f) is spayed or neutered (if 9 months and
older) and not in heat. I acknowledge that vaccinations cover a limited number of disease strains and
hereby release the Company from any liability should my dog be exposed to any diseases while on
the Premises.
I represent and warrant to the Company that I will comply with the Rules governing the Social Area,
a copy of which is attached as Schedule 1. I also agree to the Privacy Policy, a copy of which is
attached as Schedule 2.
Assumption of Risks
In bringing my dog(s) in the Social Area, I understand and assume the risks associated with and
agree to accept responsibility for any and all problems or damages that may occur, both foreseen
and unforeseen, including by way of example and not by limitation the following:
- Ingestion of materials or substrates
- Ingestion of human food and/or beverages (Dog owner(s) should never let dogs eat from
human dishes, utensils or glasses and should only feed or provide water to dogs from the
designated dog bowls provided by the Company)
- Injuries related to environments/agility equipment
- Potential encounters with wildlife including birds, biting or stinging insects or fleas and ticks
- Skin allergies/hot spots
- Torn toenails
- Cruciate tears/orthopedic problems
- Bite wounds or injuries to dogs or humans
- All other injuries that may be sustained by a dog or human
- Upper respiratory infections/possible contagious infections
- Weather related health issues
- All damage caused by my dog(s) to the Premises or personal property
I hereby consent to allow my dog(s) to receive medical treatment deemed necessary if my dog(s) is
injured or requires medical attention during my participation in the Social Area. I understand and
agree that I am solely responsible for all costs related to such medical treatment and any related
medical transportation and/or evacuation. I hereby release, forever discharge, and hold harmless the
Company from any claim based on such treatment or other medical services.
Liability/Indemnification
I acknowledge that I am liable for all actions of my dog(s) on the Premises, including but not limited
to any injuries caused by my dog(s) and damage to the Premises or personal property caused by my
dog(s). I shall defend, indemnify, and hold harmless the Company and its officers, directors,
manager(s), employees, volunteers, agents, affiliates, subsidiaries, shareholders, members,
successors, and assigns (collectively, "Releasees") against any and all losses, damages, liabilities,
deficiencies, claims, actions, judgments, settlements, interest, awards, penalties, fines, costs, or
expenses of whatever kind, including reasonable attorneys' fees, the costs of enforcing any right to
indemnification under this Release, and the cost of pursuing any insurance providers, incurred by or
awarded against the Company or any other Releasees arising out of or resulting from any claim of a
third party related to the actions of my dog(s), including but not limited to any all injuries caused by
my dog(s) and damage to the Premises or personal property caused by my dog(s), including any
claim related to my own negligence or the ordinary negligence of the Company.
I ACKNOWLEDGE AND AGREE THAT BY EXECUTING THIS MEMBERSHIP AGREEMENT, THE
TERMS AND CONDITIONS SET FORTH HEREIN, INCLUDING ALL RELEASES AND WAIVERS,
APPLY TO YOUR INITIAL VISIT TO GREEN DOG BARKERY AND TO ALL FUTURE VISITS. I
ALSO ACKNOLWEDGE AND AGREE THAT THE TERMS OF THIS AGREEMENT APPLY TO AND
BIND, IN ADDITION TO MYSELF, ALL LISTED OWNERS.
By signing this Agreement, I understand and agree that that the membership fees are non-
refundable. I also agree that my membership is not transferable.
Schedule 1
Social Area Rules
1) All dogs must have an active membership to enter.
2) Owners are fully responsible for their dog(s) and their actions while visiting the Premises.
3) Owners must clean up after their dog(s).
4) All humans must be 18 or older to enter with a dog and participate in social activities.
5) All dogs must be at least four (4) months old.
6) Dogs must be on-leash until inside the holding area where they can then be turned free to enter
the Social Area. While in the Social Area, all dogs must be off-leash which will allow them to move
freely and at their natural pace.
7) Dogs must not be left alone in the Social Area. If the Owner needs to leave the Social Area for
any reason, the dog(s) must either go with them (on leash) or remain in the Social Area with a
responsible adult that is monitoring them.
8) Dogs who are sick, in heat, or have fleas or ticks are not permitted to enter.
9) Outside toys, dog food, treats, or water bowls are not permitted.
10) Only harnesses and flat, buckle collars permitted; No prong, pinch, choke, chain, electric,
martingale, or locked collars.
11) Spay/Neuter Policy -- All dogs 9 months and older are required to be spayed or neutered with
proof uploaded to our membership portal prior to entry. Unfortunately, we are unable to make
exceptions. We understand that the choice to spay or neuter and at what age is a decision to be
made for each individual dog by the Owner in consultation with your veterinarian. As dogs reach
sexual maturity, hormonal changes occur that can alter behavior or may cause tension amongst
other dogs within the group dynamic. Dogs who are not fixed can receive too much unwanted
attention and feel “picked on”, which can result in a fight.
12) Vaccine Requirements -- All dogs entering the Social Area must be up to date on vaccinations,
including:
- Rabies
- Distemper/Parvo (DHPP/DHLPP/Da2PP)
- Bordetella
You must upload a copy of your dog’s vaccination record to our membership portal prior to entry and
must provide updated records as old records expire throughout the term of your membership.
13) Aggressive Behavior
- Aggressive behavior will not be tolerated by either you or your dog(s). Aggressive dog
behavior includes, but is not limited to, excessive barking as well as biting, mounting,
humping, and bullying. If you or your dog demonstrates aggressive behavior, you may be
asked to either relocate or leave the Premises by the Company. Offenses will be
documented. Repeat violations will result in permanent ban from the facility to protect our
employees and guests.
- The Company reserves the right to reposition a dog within the Premises or escort the Owner
and dog off the Premises if a dog is seen showing aggressive behavior. If conditions warrant,
it is possible that the dog will not be allowed back to the Premises.
- If dog fights occur, each Owner should grab their respective dog by the nape and pull them
away from the other dog(s) involved and reposition them until they calm down. Please note,
no method is perfect and the risks are still present. The Company reserves the right to
reposition a dog within the facility or escort the Owner and dog off the Premises if a dog fight
occurs. If conditions warrant, it is possible that the dog(s) will not be allowed back to
Premises.
- Dogs inherently play “rough.” However, rough play is not accepted if one or more of the dogs
aren't comfortable with the situation. If a dog is deemed to “bully” another dog, the Company
reserves the right to reposition a dog within the facility or escort the Owner and dog off the
Premises. Mounting or humping behaviors are included in this and are not tolerated.
14) Owners must retain possession of their leashes. The Company is not responsible for lost
leashes.
15) Owners are responsible to ensure their dog(s) do not cause damage to the Company’s property.
16) The Company’s staff has the right to refuse service to any guest for any reason.
17) When entering and exiting the Social Area, please open only one gate at a time, and be careful
not to allow other dogs to enter or exit with you.
18) If a human guest is visibly intoxicated and unable to walk (with a dog) in a straight line, they will
be sent home in a taxi or ride-sharing service such as Uber or Lyft.
19) The Company is not responsible for any injury, illness, or damage caused on our Premises, and
all human guests must be responsible for their dog(s). If your dog(s) causes any damage to a
person, a dog, or property, you are responsible.
20) An individual can only register dog(s) in which they have ownership and liability over. Foster
dogs, dogs owned by family members, and dogs owned by friends, must be registered by their legal
owners, and will not be permitted entry unless accompanied by a legal owner. An individual can only
enter the Social Area with up to two dogs at one time. Additional dogs require an additional person
that is assuming liability for the additional dogs. No more than two humans per dog.
21) The Company intends to make employees available to monitor and assist with the behavior of
dogs. You agree that you will comply with all instructions of employees at all times. You also
acknowledge that employees may not be available to monitor and assist with the behavior of your
dog(s) at any given time. You are always responsible for the behavior of your dog(s) at all times,
regardless of whether or not an employee is present.

Privacy Policy, Information Collection/Disclosure Policy
Privacy Policy
Green Dog Barkery, LLC ("we" or "us" or "our") respects the privacy of our members ("member" or
"you" or “your”). This Privacy Policy explains how we collect, use, disclose, and safeguard your
information. Please read this privacy policy carefully.
We reserve the right to make changes to this Privacy Policy at any time and for any reason. We will
alert you about any changes by updating the "Last Updated" date of this Privacy Policy. Any changes
or modifications will be effective immediately upon posting the updated Privacy Policy on our website
(www.greendogbarkery.com) (the “Site and Online Platforms”). You waive the right to receive
specific notice of each such change or modification.
You are encouraged to periodically review this Privacy Policy to stay informed of updates. You will be
deemed to have been made aware of, will be subject to, and will be deemed to have accepted the
changes in any revised Privacy Policy by your continued use of the Social Area and/or use of the
Site and Online Platforms after the date such revised Privacy Policy is posted.
Collection of your Information
We may collect information about you in a variety of ways. The information we may collect includes:
- Personally identifiable information, such as your name, address, email address, and
telephone number, and demographic information, such as your age, gender, hometown, and
interests, that you voluntarily give to us or when you choose to participate in various
activities. You are under no obligation to provide us with personal information of any kind.
- Information our web hosting company automatically collects when you access the Site and
Online Platforms, such as your IP address, your browser type, your operating system, your
access times, and the pages you have viewed directly before and after accessing the Site
and Online Platforms.
- Financial information, such as data related to your payment method (e.g. valid credit card
number, card brand, expiration date) that we may collect when you purchase, order, return,
exchange, or request information about our services. We store only very limited, if any,
financial information that we collect. Otherwise, all financial information is stored by our
payment processor, eTailPet, and you are encouraged to review their privacy policy and
contact them directly for responses to your questions.
- Information from third parties, such as personal information or network friends, if you connect
your account to the third party and grant the Site and Online Platforms permission to access
this information.
- Photos/Videos taken around the Premises of you and your dog(s) by our staff members or
other guests.
Data From Contests, Giveaways, and Surveys
Personal and other information you may provide when entering contests or giveaways and/or
responding to surveys.
Use of your Information
Having accurate information about you permits us to provide you with a smooth, efficient, and
customized experience. Specifically, we may use information collected about you to:
Administer sweepstakes, promotions, and contests. Assist law enforcement and respond to
subpoena. Compile anonymous statistical data and analysis for use internally or with third parties.
Create and manage your account. Deliver targeted advertising, coupons, newsletters, and other
information regarding promotions to you. Email you regarding your account or order. Enable user-to-
user communications. Fulfill and manage purchases, orders, payments, and other transactions.
Generate a personal profile about you to make future visits more personalized. Increase the
efficiency and operation of the Site and Online Platforms. Monitor and analyze usage and trends to
improve your experience with the Site and Online Platforms. Notify you of updates to the Site and
Online Platforms. Offer new products, services, and/or recommendations to you. Perform other
business activities as needed. Prevent fraudulent transactions, monitor against theft, and protect
against criminal activity. Process payments and refunds. Request feedback and contact you about your use of the Site and Online Platforms. Resolve disputes and troubleshoot problems. Respond to
product and customer service requests. Send you a newsletter. Solicit support for the Site and
Online Platforms.
We may use photos/videos of you and your dog(s) for client files, advertising/marketing purposes
including, but not limited to, print, signage, website, and social media.
I understand that I am solely responsible for my wellbeing and safety, and the wellbeing and
safety of my dog(s). Neither Green Dog Barkery LLC or employees, owners or staff are
responsible for my dog(s). I attest that I have read the Rules, and my signature below indicates
that I understand and will abide by all Rules in their entirety.
I am fully aware of the risks and hazards connected with the Social Area and hereby elect
voluntarily to participate in same knowing that said participation may be hazardous to me
and/or my property. I VOLUNTARILY ASSUME FULL RESPONSIBILITY FOR ANY LOSS, PROPERTY
DAMAGE, OR PERSONAL INJURY INCLUDING SICKNESS AND/OR DEATH, that may be sustained
by me, and which arises from my participation in the Social Area.
It is my intent that this release and hold harmless agreement shall bind my heirs, assigns and
personal representative, if I am deceased, and shall be deemed as a RELEASE, WAIVER,
DISCHARGE, and COVENANT NOT TO SUE any of the RELEASES.
In signing this release, I acknowledge and represent that I HAVE READ AND UNDERSTAND THE
FOREGOING LIABILITY WAIVING AND THAT I SIGN IT VOLUNTARILY as my own free act and
deed; no oral representations, statements or inducements not contained herein have been
made; and I EXECUTE THIS RELEASE FOR FULL, ADEQUATE AND COMPLETE CONSIDERATION
FULLY INTENDING TO BE BOUND BY SAME.
